At Shared Horizons, our Enhanced Family Care (EFC) program is built on the belief that every individual deserves a place to call home — a place where they are respected, supported, and valued as part of a family. We partner with caring, trained providers across New Hampshire who open their homes to individuals with developmental or intellectual disabilities, offering both stability and a nurturing environment.

Each home is carefully screened, and every match is made thoughtfully to ensure compatibility, safety, and shared values. Our professional team provides ongoing support, oversight, and training to ensure that both the provider and individual thrive together.
